Monthly Plan

This is your monthly plan. You will not be charged at the time of sign up. The full monthly plan price and per user fees will be charged beginning on the first of next month. Simply cancel before the 20th of this month to keep from being charged for next month's service.

Training Plan

This is your initial plan for setup, implementation, and training. You will be charged the down payment amount at sign up and the monthly installment amount will be added to your bill each month for the term of the contract.

Acknowledgement

I authorize GoodLook, Inc., to charge the bank account or credit card provided above pursuant to the amount indicated on the monthly invoice, during each billing period. This payment is for products and services provided by GoodLook, Inc. and its affiliates. A receipt for each payment will be provided to me and the charge will appear on my bank or credit card statement. I agree that no prior-notification for the payment will be provided except for the monthly invoice statement. I understand that this authorization will remain in effect until I cancel it in writing. I agree to notify GoodLook, Inc., in writing of any changes in my account information or termination of this authorization at least 15 (fifteen) days prior to the next billing date. For ACH debits to my checking/savings account, I understand that because these are electronic transactions, these funds may be withdrawn from my account as soon as the above noted periodic transaction dates. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this bank account/credit card and will not dispute these scheduled transactions with my bank or credit card company, so long as the transactions correspond to the items indicated in this authorization form. I acknowledge that Training Plans, other than the DIY plan, constitute a contract between GoodLook, Inc. and myself and even if the contract is cancelled, I am still obligated to pay the full training plan fee.
