System Issues Log
December 12, 2024 10:15am - 11:08am
Type: Full Service Interruption
Status: Resolved
Report: At 10:15am, the database server received an influx of large queries which overloaded the port capacity of the database server. This caused all new incoming requests to be delayed or denied. The outage should lasted approximately 2 minutes; however, an error with the datacenter support ticketing system caused a delay in enforcing our support protocols. This error delayed the final resolution by nearly 35 minutes.
Resolution: We are working with our datacenter vendor to ensure that the support protocols work as provided and that their staff is experienced in the escalation of critical database services. We are also reviewing a piece of legacy code that triggered the port overload.
June 3, 2024 8:40am - 8:55am
Type: Partial Service Interruption
Status: Resolved
Report: The HealthTrust Software & GoodLooking platforms were affected by a partial service interruption due to a database user locking protocol. Users may have experienced slow loading or unresponsive page requests during this time.
Resolution: The protocol was triggered by an excessive number of client report requests (i.e. - one client requesting an excessive number of large, unfiltered reports from multiple devices during a short period of time). We do not believe this was a dedicated denial of service attack. All systems are operational, data integrity has been verified, and utilization has returned to normal levels.
Outage: September 14, 2022 (32 seconds)
Outage Report:
The system was down for approximately 32 seconds due to a a disruption in the main datacenter internet connection. While the system automatically transferred to its backup location, the system was unreachable. We have identified an area of improvement and are taking steps to keep this issue from reoccurring in the future.
